THE DENNIS PATRICK FOUNDATION
CAMP/WORKSHOP SCHOLARSHIPS
Up to Four (4) $250 Scholarships to be Awarded Through December 2010
The Dennis Patrick Foundation is sponsoring scholarships for qualified school-age students who have the bleeding disorder ITP (Immune Thrombocytopenic Purpura) or arthritis and rheumatic diseases, who need financial assistance to attend an educational camp or workshop. Eligible Applicants:
1. Applicant must be a school-age student who has at least one of the following: the bleeding disorder ITP (Immune Thrombocytopenic Purpura), arthritis and/or rheumatic diseases; and
2. Applicant must maintain a “C” average in school; and
3. Applicant must use the scholarship award to attend an educational camp or workshop related to the bleeding disorder ITP (Immune Thrombocytopenic Purpura), arthritis and/or rheumatic diseases.
Submission Documents:
1. Official Scholarship Application.
2. Copy of school transcript.
3. Two (2) sealed letters of reference [physician referrals accepted].
4 Photograph.
Students should obtain a copy of The Dennis Patrick Foundation Camp/Workshop Scholarship Application and complete all general and personal information on the application. Completed applications should then be mailed to:
