THE DENNIS PATRICK FOUNDATION EDUCATIONAL SCHOLARSHIPS

Up to Four (4) $500 Scholarships to be Awarded April 2010

The Dennis Patrick Foundation is sponsoring scholarships for qualified graduating high school students for the 2009/2010 year. Please see your high school counselor for The Dennis Patrick Foundation Scholarship Application or visit www.thedennispatrickfoundation.org.

Eligible Applicants:

1. Applicant must be 2010 high school graduate; and

2. Applicant must have maintained a “B” average throughout high school; and

3. Applicant must have participated in their high school athletic sport program (UIL sanctioned) for the last two (2) years of his/her high school career; and

4. Applicant cannot have accepted a full scholarship; and

5. Applicant must use the scholarship award for higher education at a trade school, vocational school or an accredited 2-year or 4-year college the year following the award.

Submission Documents:

1. Official Scholarship Application.

2. Copy of high school transcript.

3. Two (2) sealed letters of reference.

4 Photograph.

Students should obtain a copy of The Dennis Patrick Foundation Scholarship Application from their high school counselor and complete all general and personal information on the application. Completed applications should then be mailed to:
